We are extending COVID TESTING (PCR) services to Saturdays at Clanzel Brown and Lane Wiley locations.

Monday-Friday: 9am-6pm

 Saturdays: 9am-4pm

While the spread of COVID-19 may seem to be trending down, high risk communities have been found to be of greater risk to infection.

There is increasing evidence that some racial and ethnic minority groups are being disproportionately affected by COVID-19.

Agape Family Health is partnering with the City of Jacksonville & Jacksonville Transportation Authority to provide free access to testing & vaccines in Jacksonville’s high risk communities as well as opportunities to share information on how individuals and families can lower their risk of infection, and improve overall health outcomes.

Get Your COVID vaccine

Agape Family Health (Agape) is now offering COVID-19 vaccinations to eligible populations.

Agape is a Federally Qualified Health Center, that provides medical home and primary care services to specific areas of Jacksonville at five clinic locations.

Agape also currently provides free COVID-19 testing at each vaccination location. Testing services are particularly targeted address access to care in areas north and west of the St. Johns River, and Jacksonville’s urban core.

Agape testing sites also include opportunities to share information on how individuals and families can lower their risk of infection, and improve overall health outcomes.

Wellness On Wheels Mobile Vaccination Schedule

We are extremely excited for this opportunity to collaborate with the Jacksonville Transportation Authority to expand access to the COVID-19 vaccine with the Wellness on Wheels program. This collaboration is a step forward in our ongoing effort to eliminate the spread of the virus and save lives.

The Agape-JTA Wellness on Wheels mobile vaccine program is a win for the entire community, and allows us to go where people are to not only to provide access to vaccine, but to build trust, respond to needs and take action.

Get Your COVID Testing

Please bring a valid form of ID.

Testing is available for those 5 years old and Up.

Our staff work very hard to have your results delivered to you within 48-72 hours.

  • If your test results are POSITIVE, you will receive a phone call from Agape Family Health within 48–72 hours.
  • If your test results are NEGATIVE, you will receive an email from Agape Family Health within 48–72 hours. If you do not have an email, you will receive your NEGATIVE results via US Mail.

Absolutely. We have many cases where a single asymptomatic patient still infected everyone in their household.

Unfortunately, your child/baby is over 3 months old and doesn't meet the eligibility criteria for this program. However, we recommend exploring other programs that may be better suited for their current age.
