With a designation as a Federally Qualified Healthcare Center (FQHC), we have served the uninsured and underinsured individuals of Northeast Florida since 2016. Remaining true to our mission of providing high quality, comprehensive healthcare at a reasonable cost to everyone from birth to adulthood, we have 5 service locations throughout Duval county.
